Структура входящего потока раненых в войнах XX в. и военных конфликтах последних десятилетий характеризуется преобладанием повреждений конечностей. По сравнению с периодом Великой Отечественной войны 1941-1945 гг., частота этих ранений в условиях современных боевых действий возросла до 81,4%, а доля переломов длинных трубчатых костей конечностей - с 45,4 до 53-60%. Одной из сложных групп пострадавших с боевой хирургической травмой конечностей являются раненые, у которых имеется сочетание ампутационного дефекта одной нижней конечности и перелома костей контралатеральной конечности. Наличие ампутационного дефекта кардинально меняет алгоритм реабилитационного лечения, в том числе программу протезирования, а также требует разработки рационального подхода к выбору метода фиксации перелома на контралатеральной конечности и сроков начала двигательной активности после операции.</p><p>Цель исследования - на основе анализа данных современной отечественной и зарубежной научной литературы изучить особенности тактики хирургического лечения раненых с множественными ранениями нижних конечностей в сочетании с ампутационными дефектами одной из конечностей и переломами костей контралатеральной с учетом анатомо-функциональных последствий боевой травмы.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Поиск источников осуществлялся в ведущих отечественных и зарубежных научных базах данных (eLibrary, Cyberleninka, PubMed, Google Scholar), которые были опубликованы за период с 2005 по 2026 гг.</p></sec><sec><title>Результаты</title><p>Результаты. Ключевая проблема заключается в выборе метода остеосинтеза поврежденной контралатеральной конечности, а также в определении показаний и сроков протезирования ампутированной конечности. Наиболее оптимальным на передовых этапах оказания хирургической помощи является внеочаговый остеосинтез стержневыми аппаратами внешней фиксации в режиме лечебно-транспортной иммобилизации.</p></sec><sec><title>Заключение</title><p>Заключение. Главной особенностью лечения данной категории раненых является необходимость ранней нагрузки и начала ходьбы для сохранения двигательной активности конечностей и предупреждения развития осложнений.
