Определить частоту проявления одиночных ворсинчатых аденом по сегментам толстой кишки и выполнить сравнение молекулярно-генетических особенностей ворсинчатых аденом со слизистой оболочкой индексного сегмента и между разными сегментами толстой кишки, а также характеристиками неизмененной слизистой оболочки аналогичных сегментов толстой кишки.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Сплошное поперечное ретроспективное исследование результатов 3086 колоноскопий. В исследовании изучены 347 клеточных образцов: ворсинчатых аденом – 109, слизистой оболочки индексного сегмента – 101, нормальной слизистой оболочки – 137. Детекцию миРНК и мРНК выполняли с помощью ПЦР в реальном времени. Детекцию соматических мутаций проводили с помощью аллель-специфичной ПЦР с гидролизуемым зондом. Результаты. Общая частота ворсинчатых аденом в структуре всех выявленных эпителиальных новообразований составила 3 %. Ворсинчатые аденомы преобладали в сигмовидной кишке, в других сегментах толстой кишки их частота достоверно не различалась. Частота выявления тяжелой дисплазии была связана с диаметром аденомы. Генные мутации, аналогичные изменениям в ворсинчатых аденомах, выявлены в слизистой оболочке индексных сегментов в прямой, сигмовидной и нисходящей кишках. Оценка относительной экспрессии 9 миРНК и 9 белок-кодирующих генов, связанных с развитием КРР, в слизистой оболочке индексных сегментов и аналогичных сегментов слизистой оболочки без аденом не выявила достоверных различий. Выполнено сравнение между разными отделами толстой кишки медиан относительных уровней экспрессии наиболее интересных маркеров: генов NOX1, LGR5, MS4A12, TIMP, Ki-67 и TERT и миРНК-135b, -20a, -21, -31 и -34a.</p></sec><sec><title>Заключение</title><p>Заключение. Полученные результаты указывают на то, что осмотр и биопсия неизмененной слизистой оболочки даже в пределах индексного сегмента не могут дать представление о риске развития ворсинчатой аденомы.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. This study aims to determine the frequency of solitary villous adenomas by colon segment and to compare the molecular genetic features of villous adenomas with the mucosa of the indexed segment and between different colon segments, as well as to consider the characteristics of unchanged mucosa of similar colon segments.</p></sec><sec><title>Methods</title><p>Methods. A continuous cross-sectional retrospective study of 3,086 colonoscopy results was performed. In the study, 347 cellular specimens were analysed, including 109 villous adenomas, 101 index segment mucosa samples and 137 normal mucosa samples. Detection of miRNAs and mRNAs was conducted using real-time PCR. Somatic mutations were identified with allele-specific PCR and a hydrolysable probe.</p></sec><sec><title>Results</title><p>Results. The overall frequency of villous adenomas in the structure of all detected epithelial neoplasms was 3%. The frequency of villous adenomas was predominant in the sigmoid colon, while in other segments of the colon their frequency did not differ significantly. The frequency of severe dysplasia was associated with the adenoma diameter. Gene mutations similar to changes in villous adenomas were detected in the mucosa of index segments in the rectum, sigmoid colon and descending colon. Evaluation of the relative expression of 9 miRNAs and 9 protein-coding genes associated with the development of CRC in the mucosa of index segments and similar mucosal segments without adenomas did not reveal significant differences. The medians of relative expression levels of the most relevant markers – NOX1, LGR5, S4A12, TIMP, Ki-67, and TERT genes, as well as miRNA-135b, -20a, -21, -31, and -34a – were compared between different colon sections.</p></sec><sec><title>Conclusion</title><p>Conclusion.
